Claims

Submitting a US State Car Insurance claim is quite easy, here's what you need to do:

Submitting an insurance claim must include complete documents related to US State Car Insurance and must be given to US State Car Insurance no later than 30 days from the event/disaster that befell the insured, for example the date of the accident, the last date of treatment, date of death, or whatever any events related to the conditions insured in the insurance policy.

To make a claim, you only need to fill out a claim form and complete the required documents according to the type of claim submitted.

To get the customer form, please contact the nearest US State Car Insurance Customer Service. The following are the document requirements based on the type of claim:

Hospital Inpatient Claim

  • Police copy
  • Photocopy of personal identity (Identity Card/passport) of the policy holder & the insured
  • Copy of family card
  • Health claim notification form / daily cash compensation
  • Power of attorney
  • Doctor's certificate regarding bodily injury or illness
  • Original Receipt & Hospital treatment fee details
  • Other supporting documents deemed necessary by the insurer
  • For products with the benefit of only inpatient compensation, you can only attach a photocopy of the legalization of receipts & details of hospital fees

Claim Death due to illness / accident

  • Photocopy of personal identity (Identity Card/passport) of the policy holder & the insuredvDeath claim notification form
  • Power of attorney
  • Death certificate from the competent authorityli
  • Doctor's certificate regarding the causes of death of the insured
  • Certificate from the police in the event of a traffic accident or other events that are reasonably suspected of having a criminal element

Accident Claim

  • Police copy
  • Photocopy of personal identity (ID card/passport) of the policy holder & the insured
  • Accident claim notification form
  • Power of attorney
  • Doctor's certificate regarding bodily injury or illness
  • Original Receipt & Hospital treatment fee details
  • Other supporting documents deemed necessary by the insurer

Critical Illness Claim

  • Police copy
  • Photocopy of personal identity (ID card/passport) of the policy holder & the insured
  • Critical illness claim notification form Power of attorney
  • Doctor's certificate regarding bodily injury or illness
  • The results of the necessary examination according to the critical illness suffered
  • Other supporting documents deemed necessary by the insurer.
